Summary of the Southport inquiry: phase 1 report

The Southport inquiry was commissioned by the Home Office following the attack by 17-year-old Axel Rudakubana at a children’s dance club in Southport in July 2024. The aim of the inquiry is to develop a clear understanding of how the attack was able to happen and to identify lessons to minimise the risk of acts of extreme violence in future.

The inquiry is split into two phases. The phase 1 report sets out to establish a definitive account of the attack and the events leading up to it, and to review the decision making and information sharing by the agencies involved.

This briefing summarises the fundamental problems identified in the phase 1 report, including:

  • the failure of any organisation or multi-agency arrangement to take ownership of the risk
  • poor information management and information sharing between and within agencies
  • excusing harmful behaviour on account of the presence of autism
  • limited oversight and intervention around online behaviour
  • the role of parenting.
